Services connected is the heart of Outreach Connected, and aims to bridge the gap between existing and available supports for service users across organisations in the community.

Services connected works alongside existing supports including health, legal, financial, employment and community organisations to ensure continuity of support to reduce disengagement from services.

Services connected also acts as an entry point into primary support services and advocates for ongoing support and engagement for the most vulnerable. This service aims to give a voice to marginalised communities and allows them a platform to be educated on the resources available to them.
